In the past I put out a spreadsheet on the wiki and asked people to
fill it in.  I'm asking for volunteers 2-3 people to help build the
questions.
 
This year it was decided, which I agree to, we will put out a google
form for everyone to enter their data and submit.  Then we will post the
data in spreadsheet form to the wiki.  The wiki is password protected so
your information is secure.
 
I was given approval by TASLA leadership to ask for some assistance
from some people to build the questions and then I or someone on the
committee will build the form and send it out to the membership.
 
I'm attaching a txt document of the questions that have been on the
spreadsheet in the past.  If there are other questions you would like to
see please send them to me.
 
I'd like to get this done by Thanksgiving if possible. - yes 3 weeks.
